2 Effective Date: 01/15/2026 Revision: 03 Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format Revolutionary FAR Overhaul (RFO) in Federal Acquisition Regulation (FAR) subpart 12.202, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-06. This solicitation is set aside for SDVOSB business concerns. Statement Of Work CONTRACT TITLE: Elevator Inspections & Certification PLACE OF PERFORMANCES: VA San Diego Healthcare System 3350 La Jolla Village Dr. San Diego, CA 92161 PERIOD OF PERFORMANCE: One base year and four (4) option years. CONTRACT TYPE: Firm Fixed Price BACKGROUND: This contract is for semi-annual inspections and maintenance review, annual/CAT 1 inspections, and safety testing of Government owned Vertical Transportation Units (VTUs) at the VA San Diego Healthcare System, to include elevators, dumbwaiters, wheelchair lifts, and cart lifts. All VTUs additionally require a 5-year/CAT 5 elevator test. GENERAL REQUIREMENTS: 6.1. The Contractor shall furnish all equipment, expertise, incidentals, labor, materials, parts, supervision, supplies, tools, transportation, and qualified personnel required and necessary to conduct inspection services in accordance/compliance with manufacturer guidelines/recommendations, current code(s), directives, regulations, Joint Commission, and industry standards at VA San Deigo Healthcare System. 6.2. Contractor shall check in with Electric Shop Supervisor or appointed prior to performing any work. 6.3. Contractor and employees shall be issued temporary ID badges from the VA Police Department that must always be worn while on station. 6.4. All damages incurred during the performance of this scope of work will be repaired by the Contractor at no cost to the VA. 6.5. Contractor is responsible for providing and utilizing all OSHA regulated Personal Protective Equipment (PPE) while performing all inspection and testing requirements. 6.6. Contractor shall follow all VA regulations and provide the means to meet these regulations. SPECIFIC REQUIREMENTS: The Contractor shall provide independent elevator inspection services that include inspection, verification, and certification of annual and semi-annual inspection, semi-annual maintenance reviews, safety tests, and 5-year load testing to ensure reliable and continuous safe operation of Government owned VTU' s. A list of all VTUs can be found in SUPPLEMENT 1 - MASTER ELEVATOR INVENTORY (See Section 8). The list is subject to change throughout the performance of the contract and all parties involved will be notified of any changes within five (5) business days. 7.1. Prior to performing independent inspection services, The Contractor shall provide notification to the COR and the Government Elevator Maintenance and Repair Contractor of the dates of proposed services. The notification should reach the Government and Elevator Maintenance and Repair Contractor at least fifteen (15) calendar days prior to the proposed inspection. The notification shall be in writing and shall include the proposed inspection and testing dates, estimated time of arrival and completion of services, name and local contact information of the technician performing the services. 7.2. Inspections and tests shall be completed in accordance with the most current edition of the American Society of Mechanical Engineers (ASME) 17.1 Safety Code for Elevators and Escalators, ASME 17 .2 Guide for Inspection of Elevators, Escalators, and Moving Walks and ASME 17.3 Safety Code for Existing Elevators and Escalators. In addition, the Contractor shall be aware of and adhere to all applicable Federal, State, County and City, rules, recommendations, regulations, guidelines and laws pertaining to the execution of all inspection services. 7.3. The Contractor shall provide the necessary labels, metal tags or other tagging and labeling according to requirements. 7.4. The Contractor is responsible for ensuring services are provided by qualified and competent personnel for all Elevator Inspections pertinent to this contract. At a minimum, contractor personnel shall maintain a CURRENT ASME QEI 1 Certification. INVENTORY: The Contractor responsible for providing required inspection for all VA San Diego Medical Center VTU s according to SUPPLEMENT 1 - MASTER ELEVATOR INVENTORY: Bldg 1 P1, P2, P3, P4, P5, P6, S1, S2, S3, S4, Research, dumbwaiter 12 MRI P1 1 Bldg 11 P1, P2, P3, P4, S1 5 Bldg 26 P1, P2, P5, P6, S1, S2, S3, S4 8 Bldg 27 P1, P2 2 Bldg 28 PG1 1 Bldg 31 PG2 1 Bldg 32 PG1, PG2, PG3, PG4 4 Total � 34 DOCUMENTATION AND REPORTS: Within ten (10) business days of the completion of an inspection or test, the Contractor shall complete and submit a separate written narrative report for each VTU inspected. The report shall include the date and time of the inspection or test, the location, the equipment, the type of service performed, actual hours spent performing requested service, and the name of the technician performing the service. The Contractor shall submit two (2) copies of each report in written format and one (1) copy in digital format to the designated COR of the facility inspected/tested, and the Elevator Maintenance Contractor of the facility inspected/tested on VA supplied forms. 9.1. The report shall identify each VTU inspected or tested by building, serial and unit number. The report shall describe any needed adjustments, repairs, worn parts and other items required or recommended for keeping the-VTU operating safely. The report shall make specific reference to safety code violations, including the title of the code being cited, the section or clause which refers to the specific violation and other reference that will assist the COR and Elevator Maintenance and Repair contractor in making necessary repairs or adjustments. 9.2. The report shall adhere to the reporting requirements of the publications listed in VA supplied forms and describe the general condition; state of repair and cleanliness of each VTU. The report shall describe the state of the surrounding pits, access, lighting, machine rooms, cabs and landings. In addition, the report shall include any Contractor recommendation(s) that will assist the Government in maintaining the equipment in optimum operating condition. 9.3. The contractor shall fully complete and submit with report one (1) digital and two (2) paper copies of the most current version of SUPPLEMENT 2 - Checklist for Inspection of Electric Elevators and SUPPLEMENT 3 - Five (5) Year Full Load Test Inspection In lieu of the ASME approved forms listed above. 9.4. During an inspection or test if the contractor discovers a defective VTU that requires immediate repair or service, he/she shall notify the COR and the Elevator Maintenance and Repair Contractor prior to leaving the facility where the defective VTU is located. The Contractor shall describe the location of the defective VTU, repairs or adjustment needed, and if the VTU can or should remain in service. Contractor shall follow-up in 24 hours with a written report of the defective VTU and describes the action taken to report needed repairs. CONTRACTOR'S QUALIFICATIONS: Contractor's personnel performing on this Contract must be fully qualified competent service technician(s). "Fully qualified" is defined as meeting the requirements of the most current edition of ASME QEI-1 for the Qualification of Elevator Inspectors. The Contractor will be required to update competency information throughout the life of the contract as changes occur. The Contractor must provide for Government approval credentials of each service technician. Credentials must be provided prior to the employee being assigned services responsibilities on Government equipment listed herein. Contractors shall provide proof and adequacy of past performance history, references, and required qualifications. This list is· not an all-inclusive list, therefore it is the responsibility of the contractor to ensure all applicable standards, policies, regulations, and/or governing bodies not listed here are adhere to. Furthermore, contractor is responsible to adhere to all current addition of applicable standards, policies, regulations, and/or governing bodies. 11.1. NFPA 70E -Standard for Electrical Safety in the Workplace SAFETY REQUIREMENTS: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. The VAMC San Diego building 1, main building, contains asbestos. Any worker entering these asbestos containing areas must have an asbestos certification of no less than 16 hours and a valid fit test certification. Certifications must be presented to the VA project manager before any work in an asbestos area may begin. CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. PARKING POLICY: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. IDENTIFICATION BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. SMOKING POLICY: Smoking is not permitted within the VA Healthcare System facilities campus. NORMAL WORKING HOURS: Normal hours of operation are Monday through Friday from 08:00 am to 4:30 pm Pacific time. All work is expected to be performed generally during normal hours of coverage unless requested or approved by the COR or his/her designee. Before commencement of work, the Contractor shall confer with the VA COR and Contracting Office to agree on a sequence of procedures; means of access to premises and building. All work performed, to include delivery of materials and equipment, shall be made with minimal interference to Government operations and personnel. Work performed outside the normal hours of coverage must be approved by the COR or his/her designee. OVERTIME AND NATIONAL HOLIDAYS: Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.
