Location: Warm Springs Health & Wellness Center, 1270 Kot-Num Road, Warm Springs, OR 977761 Scope of Work: Procure, deliver, remove existing and disposal, install, and verify operation of a Maestro2 Optical Coherence Tomography with Angiography (OCTA) system with table at Warm Springs Health & Wellness Center. Includes software and upgrades (if Applicable), removal and disposal of existing OCTA equipment, installation, calibration, warranty, documentation, 24/7 support (if applicable), on-site training, optional service contract coverage, and HER/PACS integration. Includes compliance with manufacturer specifications, safety and HIPAA requirements, and all labor, material, and equipment, and transportation necessary for full delivery and installation. Performance Period: Date of award through one year of contract. The performance period includes delivery, removal of existing equipment, installation, testing, and final acceptance.
