Submit questions by email to vance.farrell@va.gov . Addendum to FAR provision 52.212-1, Instructions to Offerors: Each offer shall include: Cover Letter completed by the offeror (see Exhibit A). Vendor s Questionnaire for Technical Evaluation completed by the offeror (Exhibit B). Past Performance Information Copy, or draft of Emergency Plan (SOW, paragraph 12) Copy, or draft of Quality Assurance Program Plan (SOW, paragraph 15) Completed/signed VAAR 852.219-75, Notice of Limitations on Subcontracting-Certificate of Compliance for Services and Construction Nonfunctioning pro-rated monthly Rental Fee rate (SOW, 2.c.) Failure to provide all required documentation will result in the offer to be determined noncompliant. Offers with missing documents will not be evaluated. NCO-17 does not intend to enter into discussions with all offerors so that one or some can provide missing documents. Instructions for Technical: Technical Submission and Qualifications: Specifically stating how the offeror intends to meet the requirements in the Statement of Work (SOW). A technical description of how the quote is offered in sufficient detail to evaluate compliance with the requirements in the solicitation. Provide: Offeror shall attest with their offer that the operators are or will be licensed. Actual operator licenses shall be provided at the contract kick-off meeting. Offerors shall attest with their offer their proposed Technologist s competency. Technologist competency documents shall be provided at the contract kick-off meeting. Credentials and qualifications for the job. Current competency assessment (assessment of knowledge, skills, abilities, and behaviors required to perform the job correctly and skillfully; includes age-specific knowledge and skills required to provide care for certain patient populations, as appropriate). Evidence of relevant continuing education during the past two years. Offeror shall attest with their offer a description of Health examination records of all individuals performing work under the contract. Actual or copies of health records shall be provided at the contract kick-off meeting. Annual TB skin test and recent (within the last year) chest X-ray if there is a positive TB skin test. Hepatitis B immunity (hepatitis immune titer, if provider has had the series of shots; if no immunity, evidence that the provider has started the hepatitis B vaccination series). Varicella titer if the provider has not had chicken pox and has direct patient contact. Emergency Plan Quality Assurance Program Plan Submit a description of the proposed MRI (SOW, 4.b.) and trailer (SOW, 8.b.) to include make, model and specifications. Instructions for Past Performance: Offeror should provide at least two (2) references (maximum of 5) of similar contracts in government, public or private sector. Provide contract numbers, point of contact name and phone numbers, magnitude and period of performance. Instructions for Price: Offer may complete the schedule in the SF1449 or provide pricing information in their own format (Microsoft or PDF document). Quotes must be valid for at least 60 days. Offeror must currently be certified by the SBA as a Service-Disabled Veteran-Owned Small Business (SDVOSB) Offeror must be registered in SAM and have completed FAR provision 52.212-3. This solicitation includes FAR provision 52.237-1, Site Visit. A formal site visit is not scheduled. Addendum to FAR provision 52.212-2, Evaluation: Evaluation approach: the government will evaluate offers using the comparative evaluation of offers outlined in RFO 12.203(c)(2), where offers will be compared to one another to determine which provides the best benefit to the government. The government reserves the right to consider an offer other than the lowest price that provides additional benefit(s). Offers may exceed the minimum requirements of the solicitation. The government reserves the right to select an offer that provides a benefit to the government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The government is not requesting or accepting alternate offers. Factor 1: Technical Technical Factors: Offeror shall attest with their offer that the operators are, or will be, licensed, competent, and sufficient health examination records. Emergency plan addressing emergencies such as patient codes, personal injury, fire, and disruptive behavior. Quality Assurance Program Plan incorporating the minimum requirements in paragraph 15 in the SOW. Plan must demonstrate a management approach for the offeror establishing a high degree of confidence, innovation, and low-performance risk. MRI specifications of the mobile unit. Factor 2: Past Performance Recent (last 24 months), relevant, and successful current and past performance in the performance of MRI and technologist services commensurate with the solicitation magnitude of services required in the SOW. Past Performance submissions will be evaluated to determine the competency of the offeror s historical performance and how that performance can predict the offeror s potential for performance of the tasks in the solicitation. The government may review past performance information from various sources, including databases. Factor 3: Price The government will evaluate price by adding the sum of all priced line items, including all options. Prices will be evaluated for completeness and reasonableness. Unrealistically high or low offers, initially or subsequently, may be determined for eliminating an offer from competition, either on the basis that the offeror does not understand the requirement, or has submitted an unrealistic offer. Additionally, unbalanced pricing poses an unacceptable risk to the government and may be reason to reject the offer.
